1. Has Your Thyroid Actually Been Tested — Fully?
Autoimmune thyroid disease is more than twice as likely in PCOS as in the general population, according to a 2024 meta-analysis of 18 studies covering 3,657 participants, which found significantly higher odds of autoimmune thyroiditis in PCOS (odds ratio 2.38) alongside elevated thyroid-stimulating hormone and thyroid antibody levels. An underactive thyroid slows metabolic rate directly, and it is common enough in PCOS that a single old TSH result from years ago is not enough to rule it out today.

2. Is Your Ferritin Actually Adequate, Not Just Your Hemoglobin?
Heavy or irregular bleeding — common in PCOS — depletes iron stores well before hemoglobin drops low enough to register as anemia on a standard blood count, and low iron without anemia still causes fatigue and reduced exercise capacity. A Cochrane review of 67 randomized trials in over 8,500 menstruating women found that correcting iron status measurably improved exercise performance and reduced symptomatic fatigue — both of which directly affect how much a person can move in a given week, independent of intention.

3. Have You Been Screened for Sleep Apnoea?
Obstructive sleep apnoea shows up in roughly 37% of people with PCOS compared with about 6% of people without it, according to a 2025 systematic review and meta-analysis of eight studies that directly informed the 2023 international PCOS guideline’s recommendation to screen for it. Untreated sleep apnoea disrupts the hormones that regulate hunger and blood sugar overnight, and it is under-diagnosed specifically because loud snoring and daytime exhaustion get written off as “just tired,” especially in people who are not the stereotypical sleep-apnoea profile.
A sleep study is the only way to confirm or rule this out — a symptom checklist alone under-detects it. If you wake unrefreshed regardless of hours slept, or your partner has mentioned gasping or breathing pauses, raise it explicitly rather than waiting for it to come up on its own.
4. Could a Medication Be Working Against You?
Several medication classes are well documented to promote weight gain or blunt weight loss as a side effect, independent of anything the person taking them is doing differently — certain antidepressants, some antipsychotics, some anti-seizure medications, corticosteroids, and some forms of hormonal contraception are the most commonly implicated categories in general clinical literature. This is not a reason to stop a medication unilaterally; several of these treat conditions where the medication’s benefit clearly outweighs a metabolic side effect. 5. Are You Under-Eating, and Has Your Body Adapted to It?
Cutting calories works until the body compensates for it, and that compensation is measurable, not imagined. In a well-known follow-up of contestants from a televised weight-loss competition, resting metabolic rate dropped by 610 kcal per day by the end of a 30-week program — and six years later, despite most participants regaining a substantial portion of the weight, resting metabolic rate remained roughly 500 kcal per day below what would be predicted for their body composition and age. That reduction is called metabolic adaptation, and it persists long after a restrictive period ends.

6. Is the Scale Measuring Fat — or Water and Glycogen?

9. Is PCOS Actually the Reason at All?
