Vegetarian and Vegan Diets With PCOS: Hitting Protein Without Meat

The short answer
Vegetarian and vegan diets are not linked to worse PCOS outcomes — one study of 144 women with PCOS found lower testosterone in vegetarians than non-vegetarians. The real risk is four nutrients that measurably run low on a plant-based diet — B12, iron, omega-3 and zinc — not protein itself.
Is a vegetarian or vegan diet good for PCOS?
Yes, on the evidence available — provided protein and four specific nutrients are actually planned for, not assumed to work themselves out.
Removing meat does not automatically improve or worsen PCOS. What changes PCOS markers is what replaces the meat. Swap it for legumes, tofu, tempeh and a plan for the nutrients below, and the picture looks favourable. Swap it for more rice, bread and potato because that’s what’s already on the plate, and you can end up with a diet that is lower in androgens on paper but higher in refined carbohydrate in practice — a trade that is not obviously a win.
What actually changes on a vegetarian diet in PCOS?
A 2019 case-control study compared 82 women with PCOS following a vegetarian diet in New Delhi against 62 women with PCOS following a non-vegetarian diet in Srinagar, matched for age and BMI, alongside healthy comparison groups from each city. Vegetarian women with PCOS had lower Ferriman-Gallwey hirsutism scores, lower total testosterone, and lower serum lipid levels than their non-vegetarian counterparts.
The same study also found something the “just go vegetarian” version of this advice leaves out: vegetarian women, with and without PCOS, had higher pro-inflammatory markers (TNF-α, IL-6, IL-1β, hs-CRP, resistin) and lower anti-inflammatory markers (IL-10, adiponectin) than the non-vegetarian groups. The vegetarian cohort in this study also derived a higher percentage of its calories from carbohydrate. The authors attributed the inflammation difference to overall dietary composition rather than to avoiding meat itself — which is the same point this article keeps returning to: the label is not the mechanism.
The real problem: protein without leaning on refined carbohydrate
When meat leaves a meal, the path of least resistance is more of what’s already there — rice, bread, pasta, potato — because those are the foods most plates are already built around. That raises the glycaemic load of the meal (see how rice and pasta portions affect glycaemic load) without adding the protein meat used to supply. Hitting a real protein target requires a deliberate swap, not a subtraction.
You also do not need to combine specific proteins at the same meal to get complete amino acid coverage — that is outdated advice. What matters is enough total protein across the day, from enough variety of sources that the amino acids one food runs low on, another one covers. The legumes on this list — lentils, chickpeas, black beans — are each low in the amino acid methionine, and grains tend to be low in lysine. Eating both food groups across the same day, not necessarily the same meal, closes that gap. Tofu, edamame, seitan and quinoa each count as a complete protein on their own, so they carry less of a planning burden than legumes do.
Which nutrients actually run low, and how are they tested?
Four nutrients account for nearly all of the legitimate nutritional concern with a vegetarian or vegan diet, and each is directly testable rather than a guess.
Vitamin B12 exists in essentially no plant food in a form the body can use, because it is made only by bacteria, not plants. A 2021 systematic review of 141 studies found vegans had the lowest vitamin B12 intake and status of any diet group compared, alongside the lowest calcium and iodine intake and lower bone mineral density. A standard serum B12 test is the starting point; if it comes back borderline-normal, ask for a methylmalonic acid or active-B12 (holotranscobalamin) test, since serum B12 alone can look falsely reassuring. Fortified foods or a direct supplement are the only reliable fix — there is no whole plant food that substitutes for this one.
Iron absorbs less efficiently from plants than from meat, and PCOS’s irregular or heavy bleeding patterns add to what a body needs to replace. Vegetarians run an average serum ferritin 29.71 µg/L lower than non-vegetarians (95% CI: −39.69, −19.73) across a meta-analysis of 24 cross-sectional studies — a gap more pronounced in men (−61.88 µg/L) than in premenopausal women (−17.70 µg/L), though still present in both. A ferritin blood test is the relevant one, not haemoglobin alone, since ferritin — the body’s iron store — typically falls before anaemia becomes visible on a standard blood count. Omega-3 (EPA and DHA) is where plant sources genuinely fall short rather than just testing lower. Plant omega-3 is alpha-linolenic acid (ALA), and the body converts ALA to EPA and DHA inefficiently and inconsistently. High-dose flaxseed or echium seed oil supplements produced no reliable increase in omega-3 index across the trials reviewed, while algal oil raised it in every study tested. An omega-3 index blood test, where available, is the direct measure; an algae-derived EPA/DHA supplement — not more flaxseed — is the fix the evidence actually supports.
Zinc is not routinely tested unless a deficiency is already suspected, but its bioavailability from plant foods is measurably lower because phytates in grains and legumes bind it. A 2009 review of vegan diets flagged iron and zinc status as a specific concern due to limited bioavailability from plant sources alongside vitamin B12, vitamin D, calcium and long-chain omega-3s. Soaking, sprouting or fermenting legumes and grains before eating them measurably improves how much zinc the body can actually use from the same food.

Who this will not work for
If your version of “vegetarian” is built around white rice, naan, sweetened yogurt and fried snacks with the meat simply removed, you can end up with a higher glycaemic load and no additional protein — worse on both counts than a mixed diet with a real protein source at each meal. The 2019 comparison study above is a real-world illustration of exactly this pattern.
If you rely on seitan as your main protein source, know that it is wheat gluten, unsuitable if you are gluten-sensitive or have coeliac disease, and low in the amino acid lysine on its own — treat it as one source among several, not the whole plan. And if you have not eaten a source of vitamin B12 in any form (fortified food or supplement) for more than a year, that is worth testing now rather than waiting for symptoms, since B12 deficiency can take years to become apparent and is not always fully reversible once nerve symptoms appear.
